Getting Started
Are you ready to take control of your health and achieve permanent weight loss? At CNC , we understand that this is a
difficult decision. If you are ready to begin, please call us at 303-750-9454 and we are happy to find a time that works
for you. Or, if you are unsure if CNC is for you, we are happy to offer a free meeting with one of our Registered
Dietitians. This will give you an opportunity to see our facility, meet our wonderful staff, and discuss whether our
programs are a good fit. We offer this session free of charge and without obligation.
Paperwork
CNC is pleased to offer both an on-line patient information form and an on-line medical and weight control history.
Please be sure to both read the New Patient Information form, and to completely fill out the medical and weight
control history (this is now accessed through the secure patient portal, allowing you not only to submit your
information, but to log in at a later date to make changes, and also to utilize the secure messaging utility).
Location
Clinical Nutrition Center is just West of Tamarac Square, across the street from Whole Foods. From I-25, head East on
Hampden to 7500 E. Hampden Avenue (just past Poplar, just before Tamarac Square) and turn Left. CNC is located on
the third floor of Building One in the Tamarac Plaza complex (7555 E. Hampden Ave). Parking is available in front of
the building and also in the parking lot on the south side, between the building and Hampden Avenue. Please call if you
need additional directions.

Fees / Insurance Reimbursement
Because we are medically supervised, some insurance carriers do offer partial reimbursement, especially if there are
other weight-related medical conditions. Also, some patients are able to apply their fees towards the deductible for their
Health Savings Accounts, or receive payment from their Flex Spending Accounts. We encourage you to discuss
coverage with your carrier to see what your coverage is, and are happy to provide you with receipts that you can then
submit to your insurance carrier.
