THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

1. Our Commitment to Your Privacy At Clinical Nutrition Center (CNC), we understand your health information is personal. Although we are a direct-billing practice, we voluntarily adopt these privacy standards to ensure your Protected Health Information (PHI) is handled with the highest level of integrity and in compliance with state and federal standards.

2. How We May Use and Disclose Your Health Information

  • For Treatment: Our physician, PAs, and RDNs share your information to coordinate your weight loss plan, lab reviews, and prescriptions.
  • For Payment: To process your direct payments, provide you with “Superbills” upon request, or manage your account in our secure system.
  • For Operations: To run our practice, ensure quality care, and train our staff.
  • Colorado Prescription Drug Monitoring Program (PDMP): Because our clinic prescribes and dispenses controlled substances (such as phentermine and diethylpropion), we actively utilize the Colorado PDMP database. We query this database prior to prescribing controlled substances to review your prescription history, ensure your clinical safety, and maintain compliance with medical board standards of care.
  • Required by Law & Family Communication: We may disclose PHI when required by law, or, with your permission, we may discuss your care with a family member or caregiver.

3. Your Rights Regarding Your PHI You have the right to review and receive a copy of your medical and billing records, ask us to amend incorrect information, request an accounting of disclosures, and request restrictions on how we use or share your information. As a direct-pay practice, if you pay for a service out-of-pocket in full, you have the right to restrict disclosure of that info to a health plan. We will notify you promptly if a breach occurs that compromises your information.

4. Security of Your Information Your data is stored on a custom, secured server physically located at our office, not on a third-party cloud. We utilize a secure patient portal for the transmission of labs and clinical messages. We will never sell your PHI.

5. Complaints If you believe your privacy rights have been violated, you may file a complaint with our office at 5995 Greenwood Plaza Blvd, Ste 150, Greenwood Village, CO 80111 (Phone: 303-750-9454), or with the U.S. Department of Health and Human Services. You will not be penalized for filing a complaint.
