Starting July 1, 2026, Medicare Part D covers Wegovy, Zepbound, and Foundayo for weight loss through the new GLP-1 Bridge program. Eligible beneficiaries pay a flat $50 monthly copay. No opt-in. No plan change. No phone call to your insurer.

What’s New: The $50 Monthly Copay
Starting July 1, 2026, eligible Medicare Part D beneficiaries pay a flat $50 per month for covered anti-obesity medications through the Bridge program. The Bridge runs through December 31, 2027 and includes:
- Wegovy (semaglutide) — weekly injection for weight loss
- Zepbound (tirzepatide) — weekly injection for weight loss
- Foundayo (orforglipron) — the new daily pill for weight loss
Coverage is automatic. You do not need to call your insurance agent, switch Part D plans, or opt in. Under the Bridge, Part D plan sponsors carry zero financial risk — CMS funds this directly.

Do You Qualify?
A provider must submit a prior authorization that attests you meet one of these BMI tiers. BMI is measured at the time GLP-1 therapy is initiated — not at the time of the prior authorization submission.

Coverage operates outside the standard Part D benefit for the duration of the demonstration. Your prescribing provider attests to the BMI criterion at the time therapy was started.

Who Does NOT Use the Bridge
The Bridge covers GLP-1 medications for the weight loss indication only. If you already qualify for a GLP-1 under a different Medicare Part D pathway, that is the route your prescription follows — not the Bridge.
You will not be routed through the Bridge if you have any of these conditions:
- Type 2 diabetes — Ozempic, Mounjaro, and other GLP-1s are covered under standard Part D for T2D. Your prescriber uses the T2D pathway, not the Bridge.
- Obstructive sleep apnea (OSA) — Zepbound is covered under standard Part D for OSA. Your prescriber uses the OSA pathway, not the Bridge.
- MASH (metabolic dysfunction-associated steatohepatitis, formerly NASH) — covered under standard Part D pathways.
If you have one of these conditions and also live with obesity, your provider will prescribe a GLP-1 for the qualifying secondary condition. The Bridge exists for patients who would otherwise have no Medicare coverage for weight loss.
What About Cost?
The $50 monthly copay is real progress. But be direct: depending on your medication and pharmacy, the list price can still add variability, especially on higher doses. CMS funds the Bridge directly — your Part D plan does not carry financial risk — but the copay is the copay.
If affordability is a concern, talk to your doctor about:
- Lower-cost alternatives — including older oral medications we keep in stock at the clinic (phentermine, diethylpropion, Contrave, and others)
- Dose optimization — staying on the lowest effective dose
- Patient assistance programs through the manufacturer
- Splitting strategies for some injectable pens at higher doses

What Happened to the BALANCE Model?
If you have followed the policy news, you may have heard of the BALANCE Model — originally meant to pair medication access with mandatory lifestyle programs.
On April 21, 2026, CMS cancelled the Medicare Part D version. Tying medication access to mandatory lifestyle hoops creates another barrier to care. CMS extended the GLP-1 Bridge through end of 2027 to ensure uninterrupted access.
The Medicaid BALANCE Model is launching separately in 2026 — a different program for a different population. If you are on Medicaid (not Medicare), ask your caseworker about your state’s coverage.
How CNC Submits Your Prior Authorization
A visit at our Greenwood Village clinic is required before we will initiate a Bridge prior authorization. We will not prescribe through the Bridge program without an established clinical relationship and a documented BMI attestation from your provider at the time therapy starts.
Once you are an active CNC patient, your care extends beyond the prescription. Between visits, you have direct access to your care team through our secure patient portal — message your provider or your registered dietitian with questions, side effects, or refill requests without waiting for an appointment. CNC patients get unlimited messaging, real answers, and a clinical team that picks up the phone.
Here is how the PA process works once you are an active patient:
- Complete a visit at CNC. Your provider confirms BMI eligibility, reviews your medical history, and documents the indication for therapy.
- We submit the prior authorization to your Part D plan with the required BMI attestation and clinical documentation.
- We track the PA through approval — most plans respond within 72 hours.
- We coordinate the $50 copay structure with your pharmacy and Part D plan.
- We refill and renew on schedule — including updated BMI attestations if your therapy plan changes.
You focus on the medicine. We handle the paperwork.
How to Start at CNC
If you are already a CNC patient: Call us at (303) 750-9454 or message the patient portal. We will queue your PA immediately for a July 1 effective date.
If you are new to CNC:
- Book a free 15-minute preview visit (in-person or telehealth) to confirm CNC is a fit for you.
- Schedule your first full visit with a CNC provider. Your provider will confirm BMI eligibility and discuss your medication options.
- We submit the prior authorization within 24 hours of your clinical decision.
- Your medication ships or is dispensed once the PA is approved.

Frequently Asked Questions
Does Medicare cover Wegovy for weight loss in 2026?
Yes. Starting July 1, 2026, Medicare Part D covers Wegovy (semaglutide) for weight loss under the GLP-1 Bridge at a $50 monthly copay. No secondary indication is required.
Does Medicare cover Zepbound for weight loss?
Yes. Zepbound (tirzepatide) is covered for weight loss under the Bridge. You no longer need an obstructive sleep apnea diagnosis to qualify.

Do I need to switch Part D plans?
No. Coverage is automatic across all Part D plans. You do not need to opt in or change plans.
Is Foundayo (orforglipron) covered?
Yes. Foundayo (orforglipron), the new daily pill, is covered under the Bridge at the same $50 copay.
Do I need a prior authorization?
Yes. Your prescribing provider must submit a prior authorization attesting to the BMI criterion at the time therapy was initiated. Clinical Nutrition Center submits these on your behalf — but only after you have completed a CNC visit.
How long does the Bridge last?
The Bridge runs July 1, 2026 through December 31, 2027. CMS may extend or modify the program; we will update this page as guidance changes.
Does CNC accept Medicare?
Clinical Nutrition Center is a direct-pay clinic and does not bill insurance, including Medicare. Your $50 copay applies to the medication, billed through your Part D plan and pharmacy. What if I am already on Wegovy or Zepbound for a different indication?
If you are on Wegovy or Zepbound for cardiovascular risk, obstructive sleep apnea, type 2 diabetes, or MASH, your medication is covered under standard Part D — not the Bridge. Talk to your provider about whether switching to a Bridge-covered weight-loss indication makes sense for you.
Do I need to visit CNC before I can use the Bridge?
Yes. A visit at our Greenwood Village clinic is required before we will initiate a Bridge prior authorization. We need to establish a clinical relationship, document your medical history, and have your provider attest to the BMI criterion at the time therapy starts. I have type 2 diabetes, OSA, or MASH. Does the Bridge apply to me?
No. The Bridge is for the weight loss indication only. If you have type 2 diabetes, obstructive sleep apnea, or MASH, your GLP-1 is covered under standard Medicare Part D through the indication-specific pathway — not the Bridge. Your prescriber will use whichever pathway matches your primary diagnosis.
